Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm including severe burns to employees in that employees were exposed to valves of a heat exchanger that did not have a working safety slide mechanism or any type of car seal, end cap or pipe plugs on the open ended valve.    a)International Paper Company, Selma, AL location:  On or about August 12th, 2015, and prior to, the employer exposed employees to burn hazards from 250 to 300 degree F. water from heat exchanger vessel SN 3968 that had a 2 inch ball valve that did not have a working safety slide mechanism or any type of car seal, end cap or pipe plugs on the open ended valve.
