29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Johnson Excavation, Inc. at 1275 Eagle Glen Rd, Steamboat Springs, CO 80487: On or before May 5, 2015 the exposing employer did not ensure that employees working inside an excavation with near vertical walls were protected from cave-in, in that employees worked at the south end of the excavation, which was approximately 10 feet in depth, without using a protective system. This condition exposed the employee to a crushing hazard caused by excavation wall cave-in.
