29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      (a)  Jobsite:  On or about August 20, 2015, three employees were working on a roof (tearing off and replacing asphalt shingles) and they were not protected from falling.  The fall hazard to the employees ranged from about 8 feet to about 25 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:    (a)  Jobsite:  On or about August 20, 2015, the employer did not retrain his employees in the knowledge and use of fall protection so as to minimize the exposure of falling from a roof.
