29 CFR 1910.217(d)(9)(i): The employer shall establish a diesetting procedure that will insure compliance with paragraph (c) of this section. a. At the workplace, the employer did not provide an established procedure to employees for setting or adjusting dies in presses, thereby exposing employees to an amputation hazard. On or about August 12, 2015 a die setter suffered a finger amputation while attempting to adjust a die while it was in Press 145.
