29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  On August 20, 2015 at the above addressed jobsite, an employee was exposed to falls while performing roofing operations from a tubular welded frame scaffold, approximately  18 feet above the ground, that did not have the working level fully planked.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  On August 20, 2015 at the above addressed jobsite, an employee performing roofing work on the side of the home was exposed to a fall hazard of approximately 18 feet, created by the presence of unprotected sides.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  On August 20, 2015 at the above address employees were performing roofing work on a residential home. Employees were not utilizing fall protection and were exposed to a fall hazard of approximately 20.2 feet.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On August 20, 2015, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three feet above the upper landing surface, thereby exposing employees to falls.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
