29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) On or about 08/18/2015 on the roof top of building#11200, employees working/walking on the roof installing insulation plates were not protected from a fall hazard, in that, fall protection was not used. An employee tripped on the edge of a skylight, the skylight broke and the employee fell 20 feet.
