29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. At a commercial job site, employees performing roofing activities such as stacking tiles in bundles on a steep roof without fall protection were exposed to fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. At a commercial job site, employees performing roofing activities accessing the roof by means of a ladder that did not extend at least 3 feet above the upper landing were exposed to a fall hazard of approximately 15 feet to the ground below.
