29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about August 13, 2015, and times prior thereto, employees framing and decking a new residential home were working at a height greater than six (6) feet without the use of a personal fall arrest system. This exposed employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about August 13, 2015, and times prior thereto, employees engaged in floor joist installation and decking activities at a height greater than six (6) feet were not in use of a fall arrest system. The employer failed to evaluate the fall hazards and take steps to reduce the risk of falls.
