29 CFR 1926.1207(c): The training did not establish employee proficiency in the duties required by this standard and did not introduce new or revised procedures, as necessary, for compliance with this standard: On or about August 15, 2015, an employee entering the permit required confined space of the bowl of Mill 1B without a permit had not been evaluated to establish his understanding of the training he had received from the employer on such entries.
