29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall:    a) At the facility employees worked with contact hazards such as, but not limited to grinders, power tools, fuel powered engines, and were exposed to contact and inhalation hazards when working with chemicals such as, but not limited to fuels, oils, grease, transmission fluids, hydraulic fluids, anti-freeze, cleaners, and degreasers. The employer had not conducted a workplace hazard assessment to determine the hazards of the job tasks and the personal protective equipment necessary to protect employees exposing them to inhalation and contact hazards.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.    a) On or about August 7, 2015, employees used employer provided fire extinguishers in an attempt to put out a gasoline fueled truck fire. The employer had not provided employees with the required fire extinguisher training. An employee was hospitalized with injuries that resulted from their attempt to use a fire extinguisher to put out the fire.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    a) At the establishment where employees were exposed to contact and inhalation hazards when working with chemicals such as, but not limited to, oils, grease, transmission fluids, hydraulic fluids, anti-freeze, cleaners, and degreasers and gasoline, a written and implemented hazard communication program was not in place.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a) At the establishment, employees exposed to contact and inhalation hazards from the chemicals they used had not received the required information and training on those chemicals. The chemicals included, but were not limited to, oils, grease, transmission fluids, hydraulic fluids, anti-freeze, cleaners, and degreasers.
