29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Multi-purpose Room/building: On or about August 20, 2015, employees were exposed to a fall hazard of over 6 feet to the dirt ground below, while using a job made ladder to access the roof, of the building under construction, which did not extend at least 3 feet above the roof's landing. The employer failed to train and instruct employees in the recognition and avoidance the unsafe work practices. b) Multi-purpose Room/building: On or about August 20, 2015, employees were exposed to a fall hazard of over 6 feet to the dirt ground below, while engaged in roofing activity. The employees were decking the roof, of the building under construction, while not being tethered to a personal fall arrest system via a lanyard. The employer failed to train and instruct employees in the recognition and avoidance the unsafe work practices.
