29 CFR 1926.1431(g)(1)(i): Hooks used in the connection between the hoist line and the personnel platform (including hooks on overhaul ball assemblies, lower load blocks, bridle legs, or other attachment assemblies or components) must be: 29 CFR 1926.1431(g)(1)(ii)(A): A bolt, nut and retaining pin, in place; or a. At the front of the building where employees were hoisted in a personnel platform, the overhead hook which had the fall protection connected in the hook did not have a bolt, nut or retaining pin to prevent a spin out of the hook.

29 CFR 1926.1431(k)(10)(i): Except over water, employees occupying the personnel platform must be provided and use a personal fall arrest system. The system must be attached to a structural member within the personnel platform. When working over or near water, the requirements of �� 1926.106 apply. a. At the front of the building where employees were working from a personnel platform to install Knife plates on the inside of a burned out building, the fall protection system was attached to a sling attached outside the personnel platform thereby exposing employees to a fall hazard.
