29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used: On August 19, 2015, on the ADA ramp to the exits on the north side of the structure, the outside insulation was pulled back from the receptacle on a flexible cord that supplied 220 VAC to an electrical spider box. Pacific Southwest employees used 110 VAC bandsaws and drills, powered from the spider box, to build formwork for concrete. Pacific Southwest employees were also exposed to the hazard when they used the ADA ramp to go from one side of the formwork to the other side. Employees were exposed electrical shock hazards.
