In the basement, the locks on the electrical panel had locks attached and were not identified as to who applied the lock thereby exposing employees to an electrical hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. In the sawmill where an employee was changing out the thrust barrier on the main saw, guarding such as, but not limited to, blocks were not placed under the thrust barrier to prevent the shaft from striking the employee thereby exposing the employee to a struck-by hazard.

In the basement area where a portable light was used to illuminate an area of the conveyor for welding, the wires were pulled from the light case due to a lack of strain relief thereby exposing employees to an electrical hazard.
