29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  a.)  Brooks Remodeling, Bailey's Auto Repair, Skelton WV:  On or about 08/21/15, the 120 volt 15 amp receptacle outlets in the garage were not provided with GFCI protection when used to supply power to an extension cord and Makita circular saw.  ABATEMENT CERTIFICATION IS REQUIRED.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a.)  Brooks Remodeling, Bailey's Auto Repair, Skelton WV:  On or about 08/21/15, employees laying sheeting on a 4/12 sloped roof were not protected from falling 18' to the ground.  ABATEMENT DOCUMENTATION IS REQUIRED.
