29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:      a) 37 Casselberry Way, Monroe Township, NJ 08831. Employees were exposed to eye and face injuries from flying debris when operating a pneumatic nail gun to install new roofing materials while not wearing any eye and or face protection.       Condition occurred on or about 08/22/15.

29 CFR 1926.1053(b)(1): Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:  a) 37 Casselberry Way, Monroe Township, NJ 08831. Employees were exposed to fall hazards while using a portable extension ladder to access the roof when the ladder was not extended three feet above the landing.   Condition occurred on or about 08/22/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 37 Casselberry Way, Monroe Township, NJ 08831. Employees were exposed to fall hazards when installing new roofing materials with no fall protection systems in place.    Condition occurred on or about 08/22/15.     A&C Roofing and Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 997181, citation 1, item number 2 and was affirmed as a final order on 04/30/2015, with respect to a workplace located at 117 East Maryland Avenue, Beach Haven, NJ.
