29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Worksite, 444 Woodbury Rd., Plainview, NY- Employee was not wearing head protection while working on the ground directly below employees on the roof using tools and supplies including, but not limited to hammers, nails and tar paper; on or about 8/22/15.  Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.   a) Worksite, 444 Woodbury Rd., Plainview, NY- Employees installing tar paper and insulation panels to a low slope roof approximately 16 feet above the ground were not protected from falling; on or about 8/22/15.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
