29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or the basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system: a) Employees were exposed to the hazard of falling approximately 15 feet while working from the JLG 450 AJ Series II aerial lift without wearing personal fall arrest equipment or a full body harness with a lanyard attached to the boom or basket. No abatement certification or documentation is required for this item.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Employer did not provide training to each employee in order to recognize hazards associated with working from aerial lifts and to understand procedures to control or minimize those hazards. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
