OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when employees were exposed to electrical shock hazard;      a) On or about 09/25/15 in the new utility room for the coolers; employees were exposed to contact with electrical current when the light fixture on the east wall did not have a cover exposing the fluorescent bulb.     b) On or about 09/25/15 in the utility room; employees were exposed to contact with electrical current when the light fixture did not have a cover exposing the fluorescent bulb.        c) On or about 09/25/15 in the lower milk parlor; employees were exposed to contact with electrical current when the light fixture on the ceiling of the room did not have a cover exposing the fluorescent bulb.       d) On or about 09/25/15 in the upper milking parlor; employees were exposed to contact with electrical current when the conduits on the on the NW side were missing a cover at the junction box.      e) On or about 09/25/15 in the utility room; employees were exposed to a fire hazard when nonmetallic sheathed conductors ( SJTOW ) and single conductors ( UF ) were attached to the PVC water lines with plastic cords, fiber cords, and plastic ties subjecting them to physical damage.      ABATEMENT CERTIFICATION REQUIRED

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when employees were exposed to contact with a corrosive liquid without a suitable eye wash station;     a) On or about 09/25/15 in the new utility room for the coolers; employees were exposed to chemical burns by contact with formaldehyde when the closest eye wash station was located on the west wall of the milk house approximately 50 feet away in an adjacent room separated by a hallway.           b) On or about 09/25/15 in the utility room; employees were exposed to chemical burns by contact with corrosive liquids ( Mandate Plus - a sanitizer with a pH 1 corrosive , Principal - cleaning solutions - a cleaning solution with a pH 13 corrosive ) when the closest eye wash station was located on the other side of a doorway which had a height of only 62.5 inches.      ABATEMENT CERTIFICATION REQUIRED

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees when employees were exposed to fall hazards from height;     a) On or about 08/21/15 in the Shop area; employees were exposed to a fall of 10 feet from the mezzanine area when the guardrail on this work platform consisted of a top rail (constructed of wooden 2x4) unable to withstand a side loading of 200 pounds and no mid rail or toe board.        NO ABATEMENT DOCUMENTATION REQUIRED
