29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  a)  At the jobsite, on or about 8/20/15:  Workers were using a Makita table saw withouot a guard on the blade.    Abatement certification must be submitted for this item.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the hole:  a)  At the jobsite, on or about 8/20/15:  Workers were exposed to falling 10 feet to the lower level where a stairway had been removed and the opening remained unprotected.

29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  a)  At the jobsite, on or about 8/20/15:  Employees were working in close proximity to and transiting past a guardrail that measured only 32 inches from the walking surface, and had no midrail.
