29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:      a) Job site, 1451 Benton Avenue, Pittsburgh, PA: On or about August 25, 2015, during roofing activities on a roof with an approximate 8 in 12 pitch, the employer did not ensure that employees used fall protection systems/devices where exposed to fall hazards of up to approximately 20 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:      a) Job site, 1451 Benton Avenue, Pittsburgh, PA: On or about August 25, 2015, during roofing activities, the employer did not ensure that the ladder used to gain access to/egress from the lower porch roof, was safely set up in that it was not extended at least 3 feet above landing area, nor was it secured and a grasping device provided.    b) Job site, 1451 Benton Avenue, Pittsburgh, PA: On or about August 25, 2015, during roofing activities, the employer did not ensure that the ladder used to gain access to/egress from the main house roof, was safely set up in that it was not extended at least 3 feet above landing area, nor was it secured and a grasping device provided.
