OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to a struck-by hazard while operating a press break with an unguarded foot treadle.    a) Firelands Manufacturing, LLC. 500 Donnenwirth Dr. Plymouth, OH: On or about Aug 24, 2015. Employees working with a Pacific Hydraulic Press at the South end of the warehouse located at 500 Donnenwirth Dr. Plymouth, OH; was utilizing a foot actuation pedal that had been modified in such a manner that it bypassed the shield guard intended to prevent accidental activation, exposing employees to a struck-by hazard while operating a press break with the unguarded foot treadle.    Among other methods, one feasible and acceptable method of correct this hazard is to follow the American Society of Mechanical Engineers (ASME) B11.3 - 2012, Safety Requirements for Power Press Breaks, follow the guidelines stated in Section 6.4.4 Unintended actuation by foot treadle. "When provided, a foot treadle of the type that severs an operator control shall be equipped with a means to prevent unintended press break actuation."

Firelands Manufacturing, LLC. 500 Donnenwirth Dr. Plymouth, OH: On or about Aug 24, 2015 Employees working with Wright 5 Ton Crane in the north side of the warehouse located at 500 Donnenwirth Dr. Plymouth, OH: was utilizing a pendant control box for crane operations that was not clearly marked for identification of function exposing employees to a stuck by or crushed by hazard.       Among other methods, one feasible and acceptable method to correct this hazard is to follow the American society of mechanical engineers (ASME) B30.11 2010, Monorails and Underhung Cranes. Follow the guidelines stated in section 11-4.2.4 Adjustments, Repairs, and Replacements. " Any condition disclosed by the inspection performed in accordance with Section 11-2.1 that is determined to be a hazard to continue operation shall be corrected by adjustment, repair or replacement before continuing the use of equipment" Subsection (5) "Function labels on pendant control stations and master switches that are illegible."

Firelands Manufacturing, LLC. 500 Donnenwirth Dr. Plymouth, OH: On or about Aug 24, 2015, an employee was exposed to a struck by, crushed by hazard from a falling load due to a broken latch on the hook for the Uesco Industries 5 ton crane above the plasma table on the south side of the building.       Among other methods , one feasible and acceptable method to correct this hazard is to follow the American Society of Mechanical Engineers (ASME) B30.10-2014, Safety Standard for cableways, Cranes, Derricks, Hoists, Hooks, Jacks, and slings. Follow the guidelines stated in Section 10-2.10.5: Removal Criteria. A latch that does not close the throat opening on the hook shall be removed from service is repaired or replaced.

An employee working at Firelands Manufacturing LLC. was operating a fork truck that did not have properly functioning braking system. Employee utilized reverse to bring the fork truck to a stop.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet.  a) Firelands Manufacturing, LLC. 500 Donnenwirth Dr. Plymouth, OH: On or about Aug 24, 2015, the employer failed to ensure that oxygen cylinders in storage were separated from fuel-gas cylinders in that Acetylene and oxygen containers were stored in the same location next to each other without the use of a noncombustible barrier. Storage of fuel and oxygen together exposes employee to a fire hazard.
