29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about august 25, 2015, at the above addressed jobsite, 1961 South Park Road, Hallandale, Florida 33009, the employer did not develop and implement an energy control program for employees who are exposed to electrical, mechanical, and thermal energies when operating machines such as, but not limited to, the McNeal, Model 1500 and the FMC, Model M-100 rotation molding machines.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    On or about August 26, 2015, at the above addressed jobsite, 1961 South Park Road, Hallandale, Florida 33009, an employee working near the crane of the north wall was using compressed air to clean himself. The dead-end pressure of the nozzle the employee was using was in excess of 100 pounds per square inch.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about August 26, 2015, at the above addressed jobsite, 1961 South Park Road, Hallandale, Florida 33009, the employer did not provide hazard communication training to the employees exposed to at least acetylene which is used for cutting metal and heating molds, argon which is used as a shielding gas when TIG welding, oxygen which is used as an oxidizer when cutting metal, carbon dioxide and argon mixture which is used as a shielding gas for MIG welding, and lacquer thinner which is used cleaning metal and flashing product surfaces.
