29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    (a) On the "Andy Line" - On or about August 18, 2015, two employees were working from temporary aluminum platforms set across an empty dip tank. The temporary platforms were not provided with a standard guardrail system, nor were the employees provided with an adequate fall arrest system. This condition exposed affected employees to a fall hazard of approximately 5'6" to the bottom of the tank.    (b) On the "Andy Line" - On or about August 18, 2015, two workers (one full time employee, one temporary employee) were working from the drip containment apron or platform at the end of the dip line next to an empty dip tank.  These workers were exposed to a potential fall hazard of 5'6" to the bottom of the tank.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    (a) On the "Andy Line" - On or about August 18, 2015, four workers were assigned the task of clearing a jam from the Andy Line. The employees were not trained to lock or tag out the line and were potentially exposed to being struck by the automated product transfer mechanism. Two of the four employees were standing on temporary platforms set on top of an empty dip tank and were further exposed to being knocked off the platforms into the empty dip tank.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department of other group:    (a) On the "Andy Line" - On or about August 18, 2015 four workers were assigned the task of clearing a jam on the line. Two of the workers were standing on temporary platforms straddling one of the dip tanks.  The other two workers were standing on the drip apron at the end of the line.  All four were exposed to a struck-by hazard and the two workers on the temporary platforms were further exposed to being knocked from the platform(s) into the tank. None of the employees had affixed a personal lockout or tagout device to the hazardous energy control point(s) on the line.
