29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure: Jobsite located at 10625 S 189th Street, Gretna, NE 68028: The employer is failing to ensure that employees engaged in residential construction activities are protected from falls heights greater than 6 feet from lower levels. On or about August 25, 2015, an employee of Romero Jimenez dba Jimenez Roofing was observed installing asphalt shingles. The employee was wearing a fall protection harness, however it was not attached to a lifeline and shock absorbing lanyard to protect the employee in the event of a fall; nor were alternative protection measures implemented.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: At the jobsite located at 10625 S 189th Street, Gretna, NE 68028: The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 6 feet. On or about August 25, 2015 the following conditions were observed by the Compliance Officer: One employee was observed exposed to fall heights of approximately 11 feet without protective measures such as but not limited to guardrails, safety nets and or personal fall protection systems. Two employees were observed using personal fall arrest systems however they were being used in a manner that would not limit the fall distance to 3.5 feet because the rope grasping device was placed at the towards the end of the lifeline
