29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  Belfast Facility - The employer did not provide abatement certification for Citation 2 Item 1 (29 CFR 1910.1200(e)(1), with an abatement date of 11/05/2015, nor did they respond to the progress report letters sent 12/18/2015 and 12/23/2015.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious phyisical harm to employees in that employees were exposed to fractures and crushing injuries due to the roll-over and/or collision of power industrial truck(s).  Throughout the facility - Seat belts were not worn by employees operating forklifts on or adjacent to uneven surfaces, loading ramps, loading docks, and other forklifts.  Seat belts in forklifts are a component of an operator restraint system that is designed to reduce the incidence and severity of injuries to the operator in the event of a roll-over accident.                                                 :

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):  Facility - The employer did not develop or implement a written hazard communications program for their employees working with among other chemicals, ammonia, LP gas, and cleaners.
