29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    1855 Boston Road, Wilbraham, MA, 01095, Creamery:    On 8/23/2015 and at times prior, the employer did not ensure that hazardous energy control procedures were utilized during service and repair of the Rotary Sqround Work Centers #8 & #10, Cherry Burrell Fruit Feeders. On 8/23/2015, an employee received a finger amputation injury while reaching into the fruit feeder rotor housing to remove accumulated materials that had jammed the machine. The 08/26/2015 OSHA Inspection revealed that operator's routinely removed the throat/chute of the fruit feeder and removed accumulated ingredients that had caused jams such as cookie dough from the fruit feeder throat/chute without following the lock out procedures for the machine.

On 8/23/2015 employees responding to a BBP clean up and BBP hazards following an injury were not identified by their job classification for this task.
