29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  In building one, the fire exit door did not have a door handle, was locked with a dead bolt, and secured shut with a strip of wood nailed into the frame the door there by exposing employees to a fire hazard.

29 CFR 1910.178(p)(1) as required by 1960.8(b):   Where powered industrial trucks were found to be in need of repair, defective, or in any way unsafe, the trucks were not taken out of service until they had been restored to safe operating condition.   In bldg. four, the employer did not ensure that a Taylor 30K forklift, Model # WPY-30-0, S/N S-40P-12767 was removed from service when found to be in need of repair.  Defects on the forklift included a non-functioning horn and no back up sensor.

29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards likely to cause death or serious physical harm, in that the employer did not ensure that the electrical equipment had listing or labeling identifying the purpose for which it was designed:  In the service bldg., employees were exposed to electrical hazards due to the fact that a 2" x 4" square electrical box with conduit knockouts was being used to power electrical hoists.  The electrical box was not designed for this purpose.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  In the Service bldg., a flexible cord attached to 110 v electrical outlet box used to power two boat hoist did not have adequate strain relief in that the flexible cord was pulled from the outlet box exposing electrical wiring there by exposing employees to electrical hazards.
