29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:     a) work site, 116 East Avenue in Warren, PA.  On or about August 18, 2015, and times prior thereto, employees were working from a ladder jack scaffold performing work activities that included, but were not limited to, removing/ replacing siding and roof work. Employees working from the platform of the ladder jack scaffold were not protected from falling 18-19 feet to the ground.

On or about August 18, 2015, and times prior thereto, employees were working from a ladder jack scaffold performing work activities the included, but were not limited to, removing/replacing siding and roof work.  The ladder jack scaffold was not erected under the supervision of a competent person as evidenced by bridging of scaffold platforms from one ladder jack to another and lack of fall protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system or provided for an alternative fall protection measure under another provision in paragraph (b) of this section:     a) work site, 116 East Avenue in Warren, PA.  On or about August 18, 2015, and times prior thereto, employees conducting residential roof work were not protected from falling at least 24 feet from the roof's eave to the ground.  Employees had installed the new trusses and roof decking.

29 CFR 1926.503(a)(1): The employer did not provide a training program, enabling each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards, for each employee who might be exposed to fall hazards:     a) work site, 116 East Avenue in Warren, PA.  On or about August 18, 2015, and times prior thereto, employees were working on the roof of a residential structure and were exposed to a fall hazard of approximately 24 feet. Employees did not receive fall protection training.

On or about August 18, 2015, and at times prior thereto, an accident prevention program consisting of frequent and regular worksite inspections, safety/health work rules, and training was not developed and implemented.  In addition, frequent and regular inspections of the jobsite, materials and equipment were not conducted by the competent person as evidenced by hazards associated with the ladder jack scaffold and lack of fall protection.
