29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: On or about August 25, 2015, employer did not ensure that the forklift backup alarm was operational. The forklift was being driven in reverse without backup alarm at a busy cross aisle area. This condition exposed employees to struck-by hazards.
