29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) Jobsite, 1200 7th Avenue, Beaver Falls, PA 15010: Observed on or about August 25, 2015, employees using a Snorkel TB80 articulating boom aerial lift (model number TB-A80RCU, serial number MR01224) to access upper levels of a three story brick structure were not wearing body belts with lanyards attached to the boom or basket. The employees were exposed to a fall hazard of approximately 50 feet to the ground below.

29 CFR 1926.352(e): When the welding, cutting, or heating operation was such that normal fire prevention precautions were not sufficient, additional personnel were not assigned to guard against fire while the actual welding, cutting, or heating operation was being performed, and for a sufficient period of time after completion of the work to ensure that no possibility of fire existed: a) Jobsite, 1200 7th Avenue, Beaver Falls, PA 15010: On or about August 24, 2015, additional personnel were not assigned to guard against fire while a propane torch was used to cut cables in an elevator shaft near the uppermost level of a three story brick building that was being demolished. The cutting operations resulted in a fire that spread to lower levels on the east side of the structure. Employees working in other areas of the building were exposed to a fire hazard.
