29 CFR 1926.451(b)(2):  Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) Left side of the building, on or about August 25, 2015 - the scaffold platform on the pump jack/ladder jack scaffold that the employee used to installing vinyl siding from was only 8 inches wide.  Scaffold deck to the ground elevation was approximately 9 feet.
