On or about August 25, 2015 a) 2nd floor Southwest elevator shaft: The elevator shaft scaffold was erected using two Aluminum outrigger I-beams with wood planks and plywood. The outrigger beams did not meet the standard's requirements. b) Southwest 7: The 2 freight elevator shaft scaffolds were erected using three Aluminum outrigger I-beams with wood planks and plywood. The outrigger beams did not meet the standard's requirements.

On or about August 25, 2015 a) 2nd floor Southwest elevator shaft: The elevator shaft scaffold was erected using 2 Aluminum outrigger I-beams with wood planks and plywood. One outrigger beam was not secured using a shoring post. An employee fell from the platform when the platform partially collapsed.
