29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    (i): A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas); and,    (ii): The methods the employer will use to inform employees of the hazards of non-routine tasks (for example, the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas:    Worksite: When employees were assigned to use caustic paint stripping chemicals, the employer did not develop and implement a written hazard communication program to inform employees of the hazards of the chemicals and how to protect themselves from the health and safety hazards of the product.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area:  Worksite: When employees were assigned to apply and abrasively remove chemical paint strippers to remove paint from a house, the employer did not provide the workers with effective information and training prior to use.  Employees incurred skin burns during use of the product

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    Worksite: When employees were required to use caustic chemical strippers to remove paint, the employer did not supply exposed employees with protective clothing that covers work clothes, and exposure for arms, neck, head.
