On or about August 25, 2015, at a jobsite located at 5651 Whipple Ave., North Canton, Ohio 44709, the employer did not protect its employees from possible impalement hazard when they did not guard the reinforcing steel onto which employees could fall.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about August 25, 2015, at a jobsite located at 5651 Whipple Ave., North Canton, Ohio 44709, the employer did not protect its employees from possible fall hazard when the ladder to access an approximately 4 foot deep trench did not extend 3 feet over its landing area.
