29 CFR 1910.23(a)(4): Every skylight floor opening and hole shall be guarded by a standard skylight screen or a fixed standard railing on all exposed sides: On or about June 26, 2015, at the above addressed jobsite, employees performing HVAC maintenance activities on a roof with unguarded skylights were exposed to fall hazards of approximately twenty-eight (28) feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about June 26, 2015, an employee was injured while performing maintenance tasks. The employee was pulling up a water hose from the ground onto the roof when he slipped and fell through an unguarded skylight on the roof of the building. The employee sustained injuries (broken arm and broken leg) that resulted in in-patient hospitalization, along with days away from work, and the employer failed to report the in-patient hospitalization within twenty-four (24) hours. There is no abatement certification or documentation required for this item.
