29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:  (a) 603 East Main Avenue Suite 2, Myerstown Pennsylvania - The employer did not provide fire extinguisher training to employees or when deciding not to have employees use fire extinguisher develop an emergency evacuation and a fire prevention program, on or about August 26, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  (a)  603 East Main Avenue Suite 2, Myerstown Pennsylvania - An employee suffered an amputation on August 4, 2015 as a result of an injury from a unguarded jointer.  The employer failed to report this work-related amputation to OSHA within 24 hours.  (The employer reported this amputation to OSHA on August 20, 2015.)  No abatement certification or documentation required.
