The employee uses a hook on portable ladder to access the steel beam and then walk the beam to reach the required areas. The employee was exposed to a fall approximately 75 inches from the top of the steel beam to the lowest portion of the ground, as determined on or about 08/24/2015.  ABATEMENT CERTIFICATION REQUIRED
