29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  (a)  Storage Area above Break Room/Office:  Employees were exposed to overhead hazards as there was no load rating posted for the storage area above the Break Room and Office where motors were being stored.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Splitter Saw: Employees were exposed to electrical and amputation hazards as a written lockout tagout program was not available onsite. Procedures were not being utilized for the control of potentially hazardous energy when the Splitter Saw for stave barrels had welding work done on the brace and the Splitter Saw was not locked out until the work was completed.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  (a) Splitter Saw Area, West Wall:  Employees were exposed to electrical shock as the wire to the 208/240 outlet did not have adequate strain relief for the electrical cord entering the box.  (b) Bandsaw Area:  Employees were exposed to electrical shock as the extension cord with an plastic outlet box to power the blower did not have adequate strain relief from the electrical cord to the box.  (c) Bandsaw Area:  Employees were exposed to electrical shock as the flexible conduit to the Cutler Hammer Control electric box did not have adequate strain relief where the conduit met the electrical box on the top of the electric box, and did not have adequate strain relief where the electrical wiring exited the base of the electrical box.
