29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer:    a) Main production facility - On or before August 26, 2015, a propane powered Clark forklift was in operation with an attachment that had not been approved by the manufacturer by means of letter or affixed label.  The attachments was a man lift that had been hand crafted from a pallet and some light gauge steel railing.  No serial numbers or make information on it.    b)  Main production facility - On or before August 26, 2015, a propane powered Clark forklift was in operation with an attachment that had not been approved by the manufacturer by means of letter or affixed label.  The attachment was a boom with a further attachment for moving wall panels once the panels were constructed.

29 CFR 1904.4(a): The employer did not record on the OSHA Form 300 or equivalent each work-related fatality, injury, or illness case that resulted in the general recording criteria:    a)  Employer's Office - On or about July 17, 2105 an employee received a laceration to the head when struck by sheet metal strapping and received hospital medical treatment.  This injury where the employee received stitches was not recorded in the injury and illnesses log because the employer was not keeping this log.
