29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area: Armstrong World Industries, Inc. - On or about August 26, the employer had not provided employees with information or training as required by this section, regarding hydrogen chloride, to which employees were exposed during the burning of the hot mix. No training was provided regarding the hazards of exposure to hydrogen chloride, how to protect against these hazards, or methods or observations to detect the presence of this chemical. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Armstrong World Industries, Inc. - On or about August 26, 2015, the employer did not provide employees with a copy of appendix D when it was determined that employee would wear Moldex 4800 N95 respirators during the off gassing of the burnt mix. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

Armstrong World Industries, Inc. - On or about August 26, 2015, the employer did not select the appropriate respirator for employees to wear while handling the burnt mix. The employer provided employees with a Moldex 4800 respirator which also provided nuisance level protection from organic vapors. This respirator did not provide protection against hydrogen chloride, being emitted from the burnt mix. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
