29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a) On or about August 25, 2015 - at a jobsite located at 39th East of King Drive, Chicago, IL. - employees were performing sewer activities in a trench measuring seven (7) feet deep, fourteen (14) feet long, seven (7) feet, five (5) inches wide that was not provided with adequate cave in protection, thereby exposing the employees to cave in hazards.
