Pump jack scaffold on northwest corner of building:  Employees were exposed to fall hazards of 15-feet, 5-inches from the pump jack scaffold platform to the ground below while installing vinyl siding where personal fall arrest systems were not being used and there were no end rails or mid-rail provided on the scaffold platform.

Pump jack scaffold on northwest corner of building:  Employees were exposed to fall hazards of up to 15-feet from the portable extension ladder to the ground below while accessing the platform of the pump jack scaffold where the ladder was not positioned so that the side rails extended at least 3-feet above the scaffold platform.
