29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. Pump jack scaffold on northwest corner of building: Employees were exposed to fall hazards of 15-feet, 5-inches from the pump jack scaffold platform to the ground below while installing vinyl siding where personal fall arrest systems were not being used and there were no end rails or mid-rail provided on the scaffold platform.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. Pump jack scaffold on northwest corner of building: Employees were exposed to fall hazards of up to 15-feet from the portable extension ladder to the ground below while accessing the platform of the pump jack scaffold where the ladder was not positioned so that the side rails extended at least 3-feet above the scaffold platform.
