29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: (a) Facility, Sheeting Department, 1600 Pennsylvania Avenue, Tyrone, PA: On or about August 21, 2015 lockout devices were not affixed to energy isolating devices by all persons involved in a maintenance task. Two persons were performing maintenance on the #3 sheeter conveyor to the case packer and only one person applied a lock to isolate the energy.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: (a) Facility, Sheeting Department, 1600 Pennsylvania Avenue, Tyrone, PA: On or about August 21, 2015 the guard for a chain and sprockets on the #3 sheeter case packer conveyor was not in place when an employee went under the conveyor to check for mechanical issues. The employee was backing out from under the conveyor when they reached for a leg on the conveyor to pull himself up and got their hand caught in the unguarded chain. The employee suffered amputations to the left middle and ring fingers down to the first knuckle. (b) Facility, Sheeting Department, 1600 Pennsylvania Avenue, Tyrone, PA: On or about August 26, 2015 a guard for a chain and sprocket on the #3 sheeter box glue area was not in place exposing employees who add pellets to the glue hopper to potential caught by hazards.
