29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) Jobsite, 3000 Horizon View Drive, Canonsburg, PA: On or about August 26, 2015, an employee was installing water lines in an unprotected excavation that was approximately 6.5 feet deep. R & B Contracting & Excavation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 576078, citation number 1, item number 3 and was affirmed as a final order on October 12, 2012, with respect to a workplace located at 2628 East Carson Street, Pittsburgh, PA 15203.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety. a) Jobsite, 3000 Horizon View Drive, Canonsburg, PA: On or about August 26, 2015, an employee was exposed to potential cave-in hazards while working in an excavation because there was not a competent person onsite to perform the daily inspections. R & B Contracting & Excavation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(k)(2), which was contained in OSHA inspection number 576078, citation number 1, item number 3 and was affirmed as a final order on October 12, 2012, with respect to a workplace located at 2628 East Carson Street, Pittsburgh, PA 15203.
