Employees gaining access to the machines who perform servicing, maintenance, and tool changes were exposed to caught in and struck by hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the following machines: a. Hardinge Polisher. The polisher did not have a chuck guard. This exposed employees to contact with rotating parts at the point of operation. b. Baldor Belt Sander. The ingoing nip point at the sanding belt and drive pulley was not guarded. This exposed employees to ingoing nip points hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.303(b)(2): Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. The employer failed to install and use metallic outlet boxes in accordance with the instructions listed and permitted uses. Knockout type boxes were installed as pendant extension cords and used in the production area. Employees were exposed to a shock hazard while handling the boxes to connect and disconnect cords. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
