On or about August 26, 2015, employees of Saul Prieto dba Prieto General Construction were observed installing asphalt shingles while exposed to fall distances of approximately 15.3 feet on the house and 9 feet on the detached garage without acceptable means of fall protection as required by this subpart.

On or about August 28, 2015; employees were exposed to unsafe ladder use in that ladders used to access upper levels did not extend at least 3 feet above the level being accessed, self supporting ladder was used to access the roof of the detached garage and employees were allowed to carry asphalt shingle bundles up the ladder to the roof.
