29 CFR 1926.1053(a)(8): A metal spreader or locking device shall be provided on each stepladder to hold the front and back sections in an open position when the ladder is being used: (a). Worksite located at 4400 S. Noland Road, Independence, Missouri- Employees were exposed to a fall hazard in that a stepladder was being used to access the roof in which the ladder's metal spreader was not extended and was leaned against the building.

Noland Road, Independence, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the roof structure did not extend at least 3 feet above the upper landing surface.
