29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) On, or about August 25, 2015 at worksite 500 W Chestnut Expressway, Springfield, Missouri, employees were exposed to hazards of electrical shock, and or, arc flash because a plastic cover on a duplex outlet was chipped and broken and did not enclose electrical contacts. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) On or about August 25, 2015 at worksite 500 W Chestnut Expressway, Springfield, Missouri.- employees were exposed to hazard of electrical shock from exposed conductors in a flexible cord which had broken free of its connection to the automated facing cancelling and sorting machine (AFCS) Machine #2. Abatement Note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
