29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.      (a)   Pacheco Framing LLC at 3835 & 3837 Utica St., Denver, CO 80212:  On and before August 11, 2015 the exposing employer did not ensure that each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system, in that employees framing the 2nd level of a residential duplex building were not utilizing a fall protection system.  This condition exposed employees to a fall hazard of approximately 12 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one handrail and one stair rail system along each unprotected side or edge:      (a)   Pacheco Framing LLC at 3835 & 3837 Utica St., Denver, CO 80212:  On and before August 11, 2015 the exposing employer did not ensure that the interior stairway used by employees to access the second level was equipped with an adequate stair rail.  The stairway had approximately 15 risers.  This condition exposed employees to a fall hazard of approximately 10 feet.
